<p> Support Worker &#8211; Telecare Response Service x 3 posts available<br />
 Location: Hertford, SG13 7HT with travel around the area<br />
 Hours: 24 per week<br />
 Contract type: Fixed Term until 30th June 2026<br />
 Salary: £19,838 per annum pro rota<br />
 Driving Requirement: Full UK Driving Licence and access to a vehicle. You will be paid from the start to the end of your shift, including travel time.</p>

<p> Are you ready to make a meaningful impact on people lives by being part of a Telecare Response Service to vulnerable adults living across Hertfordshire covering a 24 hour a day, 7 days a week rota. There is the need to be flexible, working on a rota and shift pattern of 24 hours per week. Post-holders will be expected to respond to calls by attending Service User homes within 60 minutes across Hertfordshire.</p>

<p> A day in the life of a Support Worker &#8211; Telecare Response will include:</p>
<ul>
<li>Providing practical and emotional support (telephone calls, welfare checks, shopping etc) and guidance to service users</li>
<li>Responding to calls from Service Users.</li>
<li>Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.</li>
<li>Responding to referrals from Clinicians and Community Health Professionals</li>
<li>Assess service users&#8217; needs, complete and follow a support plan for the individual.</li>
<li>Supporting service users in the weeks after hospital discharge or to prevent readmission.</li>
<li>Taking someone to a hospital appointment that they otherwise couldn&#8217;t get to or be picking up prescriptions and shopping.</li>
</ul>
<p> To be a successful Support Worker &#8211; Telecare Response, you&#8217;ll need:</p>
<ul>
<li>A full UK manual driving licence and access to a vehicle.</li>
<li>Good knowledge of services provided by the NHS and Social Care.</li>
<li>To be able to make things great. You know how to improve service quality for the benefit of users.</li>
<li>To be professional. You can deal with queries in a diplomatic and confidential manner.</li>
<li>To love being flexible. Working hours out with the norm suits you.</li>
</ul>
