<p> We are looking for a highly motivated Social Worker to start in the Joint Homelessness (Mental Health ) Team am in City of Westminster Council</p>
<p><b>Role Details:</b></p>
<p> Position Social Worker &#8211; Joint Homelessness Team (Mental Health )<br />
 Hourly Rate: £32 p/h<br />
 Location: CNWL NHS Foundation Trust</p>
<p> The Joint Homelessness Team JHT is a multi-disciplinary statutory mental health service that works with people who are homeless and sleeping rough in Westminster.<br />
 &#8211; The aims of the team are to reduce rough sleeping in Westminster by helping rough sleepers with severe and enduring mental health issues access the treatment, care, support and accommodation that they need.</p>
<p><b>About Role :</b><br />
 &#8211; To carry out social work duties within the JHT in partnership with health colleagues.<br />
 &#8211; To assess new referrals via street outreach sessions, visits to day centres and hostels.<br />
 &#8211; To carry out and record Care Act assessments where necessary to asses people&#8217;s need for care and accommodation and make the necessary referrals.<br />
 &#8211; To take a lead role in planning discharge from psychiatric hospital.<br />
 &#8211; To manage a caseload of longer-term service users in partnership with health colleagues.<br />
 &#8211; To contribute to the team&#8217;s duty system<br />
 &#8211; Hours are generally office hours with the addition of one early morning or late evening street outreach shift per month.</p>
