The purpose of the post is to assess patients needs&comma; arrange appropriate packages of care&comma; facilitate safe and timely discharges&comma; refer onto voluntary organisations&comma; identify carers needs and signpost to the most appropriate service&comma; for information and support&period; We will support you with professional supervision&comma; appraisal and training&period; The team is part of Contact Cares and benefits from also working with services in the community&period;<&sol;p>&NewLine;<p> &&num;8211&semi;
