<p><b>Job description</b>: Seven Social Care are currently seeking a highly-skilled, compassionate, and reliable Social Worker to serve in the Brent community. In this role, you will work directly with a diverse clientele to help them navigate their social, personal, and health challenges effectively.</p>
<p><b>Responsibilities:</b></p>
<p> 1. Develop and assess individualized plans to address clients&#8217; particular needs.</p>
<p> 2. Advocate for and assist clients in obtaining resources that would improve their well-being.</p>
<p> 3. Conduct comprehensive assessments of clients&#8217; social, emotional, and environmental status.</p>
<p> 4. Offer counseling and support, including therapy to individuals or groups.</p>
<p> 5. Collaborate and consult with other professionals regarding mental or physical health.</p>
<p> 6. Develop and maintain professional relationships with local, regional, and national resources and community services.</p>
<p> 7. Prepare accurate reports and maintain up-to-date client records.</p>
<p> 8. Maintain privacy and confidentiality of all client information in accordance with legal standards.</p>
<p> 9. Stay up to date with relevant legislation and policy related to social work.</p>
<p><b>Requirements:</b></p>
<p> 1. Bachelor&#8217;s or master&#8217;s degree in social work from an accredited institution.</p>
<p> 2. Solid experience as a social worker, preferably within the Brent community.</p>
<p> 3. Registration with the Health and Care Professions Council (HCPC).</p>
<p> 4. Comprehensive knowledge of social work principles and practices.</p>
<p> 5. Excellent ability to facilitate positive change and independence in clients.</p>
<p> 6. Strong communication, negotiation, and mediating skills.</p>
<p> 7. Ability to work under pressure in challenging situations.</p>
<p> 8. Demonstrated cultural competency to work with diverse client population.</p>
<p> 9. Proficiency in Microsoft Office and other case management software.</p>
<p> 10. Ability to travel to clients across Brent.</p>
