<p> Social Services directs and evaluates all aspects of the SNF Department of Social Services including admissions, discharges, referrals and family conferences.</p>
<p> <b>JOB DUTIES AND RESPONSIBILITIES:</b> </p>
<ul>
<li>Oversees overall provision of social work and discharge planning services to residents/patients and their families.</li>
<li>Coordinates admissions to the facility.</li>
<li>Actively develops, implements and evaluates departmental goals and objectives. Evaluates and monitors departmental continuous quality improvement program to insure appropriate service delivery to consumers.</li>
<li>Develops, implements and revises Social Service Policies and Procedures.</li>
<li>Advises and counsels interdepartmentally on social service issues.</li>
<li>Provides emergency on call services for mental health, drug and alcohol patients on an as needed basis when covering for the acute social worker.</li>
<li>Represents the department on various facility committees and community boards.</li>
<li>Acts as a liaison between resident/patient and community agencies including social, health and government agencies.</li>
<li>Facilitates family meetings, rehab meetings, etc., or serves as their chair.</li>
<li>Maintains adequate documentation as to comply with Federal, State and JCAHO regulations.</li>
</ul>
<p> <b>EDUCATION:</b></p>
<p> A Bachelor’s degree in Social Work or a Bachelor’s degree in a human services field.</p>
<p> <b>TRAINING AND EXPERIENCE:</b></p>
<p> One year of supervised social work experience in a health care setting working with the geriatric populace.</p>
