<p><b>OVERVIEW</b></p>
<p> Treats patients who are sick or injured, gives advice and emotional support to patients and their families, provides doctor assistance, and takes care of patient paperwork as per departmental needs and service.</p>
<p> To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. The focus of the role is both the delivery of evidence-based practice for patients presenting with long term conditions and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team and the wider community teams to meet the needs of patients, support the delivery of policy and procedures and provide leadership where required.</p>
<p> DUTIES (guideline only) </p>
<ul>
<li>Provide care to patients and residents based on their care plan outlined by their doctor</li>
<li>Accurately monitor and record observations on patients&#8217; conditions</li>
<li>Monitor patients&#8217; diet and exercise</li>
<li>Prepare patients and assist with treatments, surgery, or exams as needed</li>
<li>Participate in clinical improvements and offer ideas on clinical and non-clinical policies</li>
<li>Assist with the investigation of complaints by patients or family members</li>
<li>Manage stocks of supplies</li>
<li>Ensure storage and disposal of medication is carried out according to policy</li>
<li>Maintain accurate records of residents care, financial procedures, and medication administration</li>
<li>Identify care planning needs</li>
<li>Carry out risk assessment</li>
<li>Record patients&#8217; vital signs and medical information</li>
<li>Re-evaluate patient care plan as conditions change</li>
<li>Consult and coordinate with other healthcare team members</li>
<li>Direct or supervise less experienced nursing staff</li>
<li>Maintain continuing education and licensing requirements</li>
<li>Report any issues that arose during each shift</li>
<li>Gain the trust and confidence of each patient</li>
<li>Write patient care plans</li>
<li>Implement plans through tasks such as preparing patients for operations, wound treatment and monitoring pulse, blood pressure and temperature</li>
<li>Observe and record the condition of patients</li>
<li>Check and administer drugs and injections</li>
<li>Respond quickly to emergencies</li>
<li>Plan discharges from hospital and liaise with GPs and social workers</li>
<li>Reassure patients and their relatives and be good at communicating with them</li>
<li>Advocate on behalf of patients</li>
<li>Educate patients about their health &#8211; this may include running clinics and education sessions on topics such as diabetes, weight loss and quitting smoking- dependant on your area of speciality</li>
<li>Organise staff and prioritising busy workloads</li>
<li>Mentor student and junior nurses</li>
<li>Maintain patient records</li>
<li>Make ethical decisions related to consent and confidentiality.</li>
<li>Set up drips and blood transfusions</li>
<li>Assist with tests and evaluations</li>
<li>Carry out routine investigations</li>
</ul>
<p> NURSE SKILLS &; QUALIFICATIONS:</p>
<p><b>Essential:</b></p>
<ul>
<li>At Least 1 to 8 years Year&#8217;s Nursing Experience on Band 6</li>
<li>Current registration with NMC</li>
<li>Educated to diploma level</li>
<li>Qualified practitioner / completed a minimum of one recognised level 6 Autonomous Practice Module relating to the diagnosis and management of either injuries or illness</li>
<li>Post Registration experience Good IT skills &#8211; ability to utilise appropriate electronic systems to accurately record data.</li>
<li>Ability to work autonomously without supervision. Ability to meet deadlines and to work under pressure.</li>
<li>Ability to adapt to the demands of a constantly changing environment.</li>
<li>Aware of accountability of own role and other roles in a nurse led service.</li>
<li>Knowledge of health promotion strategies and awareness of clinical governance issues within primary care.</li>
<li>Knowledge of patient group directions and associated policy.</li>
<li>Knowledge of the NMC Code of Conduct</li>
<li>Change-management skills and ability to support patients to change lifestyle.</li>
<li>Communication skills, both written and verbal. The ability to work as a member of a large multidisciplinary team</li>
<li>The ability to communicate and interact with the public in a discreet and sensitive manner, recognising their needs for alternative methods and styles of communication Enthusiasm and a sense of humour</li>
<li>Able to work autonomously</li>
<li>Good time management skills</li>
</ul>
<p> OVERTIME</p>
<p> Payments for unsocial hours are made to NHS staff</p>

<p><b>SALARY</b></p>
<p> The salary is based on the national salary scale.</p>
<p> A Fully qualified nurses start on salaries of £31,365 rising to £37,890 on Band 6 of the NHS Agenda for Change Pay Rates. Salaries in London attract a high-cost area supplement.</p>
