Nurse Field Case Manager

Basic Function
The Nurse Field Case Manager (FCM) is responsible for a collaborative process of ongoing assessment, planning, facilitation, service coordination, evaluation, and advocacy to meet the patient /physician/client needs through flawless customer service. The Nurse Field Case Manager will deliver through expert communication skills and available resources, quality and cost-effective treatment outcomes for the patient. The Nurse Field Case Manager will work directly with the reimbursement team to help secure drug coverage.

Nature and Scope
The Nurse Field Case Manager will help support a case load in Western Canada providing disease state, molecule education and training to patients and care-givers, service coordination, in-clinic and telephone support, monitoring of adherence, and side effect management. Building strong working relationships, collaboration, effective communication and attention to detail both internally and externally are key responsibilities to meet specific team and business objectives. In addition, the Nurse Field Case Manager will attend pre-booked training sessions, complete self-learning assignments and independent studies while adhering to SDM Specialty Health Network Standard Operating Procedures, Policies, and the College of Nurses Standards of Practice.

What you will do:

Responsible for patient and physician case load

Act as the primary resource & contact for patients, physicians, Health Care Professionals and vendors.

Coordinates & facilitates the client progress from enrollment and throughout therapy

Ensure thorough documentation in patient file to ensure continuity of care is maintained and that any changes in patient care are clearly identified and transitioned accordingly

Provide status updates related to reimbursement; patient training requirements; pharmacy dispensing/ delivery/refill requests

Report all Adverse Event (AE) and Product Technical Complaints (PTC) immediately or as soon as possible upon becoming aware of the event as per current policies and procedures.

Promotes communication and coordination between the MDs/pharma/patients/clinic staff and other departments within SDM Specialty Health Network

Provide in-clinic training & build relationships on behalf of the program

Work collaboratively with the Pharma company representatives

Respect confidentiality, advocate for patients/families to facilitate positive outcomes, and ensure that ethical issues are identified and resolved according to the Canadian Nurses Association Code of Ethics

Demonstrate commitment to Continuous Quality Improvement

Maintain high standards of professional practice by adhering to the ethics of the profession and keeping abreast of the latest professional advances

Assume responsibility for personal and professional development

Other responsibilities as required by SDM Specialty Health Network

Travel is required in an established geography

What you will need:

Registered Nurse with a current, unrestricted license in the province of employment. BScN preferred, however, a combination of experience and education will be considered

Disease state experience in Multiple Sclerosis (MS)/Neuro, Rheumatoid Arthritis (RA), Gastroentrology (GI) and Dermatology is an asset; specialty biologic strongly preferred

Proficient in reimbursement landscape and completion of Special Authorization and Appeal Letters

Recent experience with Patient Support Programs (PSP)

Demonstrated customer/account focus and consistently deliver on the PSP value proposition

Fluently bilingual in both written and spoken languages is an asset (French & English)

Five or more years direct clinical experience

Strong telephony skills

Demonstrated leadership experience and ability to work under pressure

Ability to provide an organized, efficient approach to case management

Possession of excellent skills in assessment, communication, critical thinking and decision making

Ability to demonstrate understanding of SDM Specialty Health Network policies, legal considerations and professional ethics

Proficiency of computer skills including, Microsoft applications (Outlook, Word, Excel and Internet Explorer); experience with web based applications (Oracle; Seibel)

Ability to adapt to a changing environment

Commitment to the principle of quality client-centered service delivery

Knowledge of case management processes

Valid driver’s license

THE OPPORTUNITY
This position is field based in Western Canada in the Neuromodulation (NM) division. Abbott NM includes implantable devices compatible with mobile technology to help people who suffer from chronic pain and movement disorders. These non-opioid therapies allow us to provide interventional pain therapy to patients throughout the pain continuum.

The Territory Manager coordinates the activities of aligned Territory Manager and Clinical Specialists to exceed the sales plan and meet assigned administrative tasks. Applies extensive technical expertise, and has full knowledge of other related sales and clinical disciplines. Represents the organization as the prime contact with facilities, physicians and patients. Interacts with senior external personnel on significant matters often requiring coordination between organizations.

WHAT YOU’LL DO

  • Develop territory sales plans to exceed revenue and market penetration goals, ensuring collaboration and alignment with Area Vice President;
  • Responsible to generate additional sales revenue and drive Spinal Cord Stimulation and Dorsal Root Ganglion procedure penetration in an assigned region by running initiatives targeted at increasing referrals of eligible patients;
  • Identify strategies to increase referrals of appropriate chronic pain patients to interventional pain centers for Spinal Cord Stimulation, Dorsal Root Ganglion and Radio Frequency procedures and overcome barriers that are restricting the national growth and adoption of the therapy;
  • Develop and maintain relationships with new and existing customers, KOLs and industry leaders;
  • Maintain understanding of Chronic Pain Therapies in a competitive environment;
  • Work is performed without significant direction;
  • Exercise considerable latitude in determining technical objectives of assignment;
  • Coordinate implanting schedules for Territory Managers and Clinical Specialists;
  • Contact and visit potential clients in the Company’s products and addresses any client questions and concerns;
  • Collect and study information about new and existing products and monitors competitor sales, prices and products;
  • Analyze sales statistics; prepare reports; and perform required administrative sales duties, e.g., filing expense account reports, scheduling appointments, and making travel plans;
  • May attend trade shows where new products and technologies are showcased and conferences to meet other sales representatives and clients and discuss new product developments;
  • Remain current on developments in fields of expertise;
  • Manage and is fiscally responsible for consigned inventory used in the territory;
  • Resolve and/or facilitate resolution of problems including identifying causes to prevent re-occurrence;
  • Working within broad objectives, is accountable for maintaining and, where possible, expanding the level of sales in assigned area;
  • Ensure the prompt follow-up of sales leads, the investigation of complaints, and the timely completion and filing of standard reports required by sales management;
  • Have authority to make sales commitments for assigned efforts and is accountable for results. May provide substantial input to division-wide sales standards, practices, procedures and policies;
  • Comply with Health Canada regulations, other regulatory requirements, Company policies, operating procedures, processes, and task assignments;
  • Maintain positive and cooperative communications and collaboration with all levels of employees, customers, contractors, and vendors;
  • Ability to interface and interact with patients up to 50% of the time;
  • Perform other related duties and responsibilities, as assigned.

REQUIRED QUALIFICATIONS

  • Bachelor’s Degree required;
  • 3-5 years of successful sales experience, preferably within medical device industry;
  • Experience working in a broader enterprise/cross-division business unit model;
  • Well organized, capable of juggling multiple projects and accustomed to tight deadlines.
  • Excellent computer skills including MS Excel, Word, Outlook and PowerPoint;
  • Ability to work in a highly matrixed and geographically diverse business environment;
  • Ability to work within a team and as an individual contributor in a fast-paced, changing environment;
  • Ability to leverage and/or engage others to accomplish projects;
  • Strong verbal and written communications with ability to effectively communicate at multiple levels in the organization;

  • Warranty investigation, damage control and contribution to resolution
  • Sales to Engineering liaison. Ensure that customers’ expectations are crystal clear to engineering and manufacturing from quote process through build.
  • Manufacturing “red flag” immediate response and resolution.
  • Personal skills including professional appearance, time management, Morgan and competition product knowledge, various chassis product knowledge, vocational and application knowledge, proficient with Morgan IT resources, etc.
  • Other duties as assigned.

Required Credentials

  • Bachelors degree preferred in Marketing, Communications, Business Administration or related field.
  • Excellent knowledge of medium duty truck market (preferred), customer segments, and sales strategies.
  • Demonstrated expertise in product knowledge and the associated processes within the company with regard to following an order from “quote” to “delivery”, follow up service e.g. warranty, and general customer care.
  • Excellent organizational and analytical skills
  • Outstanding relationship development skills
  • Problem solving ability

POSITION OVERVIEW

The Vice President, Corporate Financing is ensuring alignment of initiatives and projects that enable the achievement of business objectives as well as corporate priorities. Leadership

Keep abreast of the general issues affecting the Corporate Financing business community including market conditions, industry practices and financial results for existing and potential clients

Optimize productivity by ensuring the proper allocation of resources for acquisition of new client relationships and retention of existing clients

Develop and implement tactical initiatives to help clients grow by adding value through knowledge, advice, and additional financing ensuring that repeat and retention goals are optimized, while balancing profit and risk

Plan and manage the visibility efforts to enhance and maximize BDC’s presence in the marketplace

Work closely with partner groups: Growth and Transition Capital and Advisory to identify businesses with significant growth potential and opportunities for BDC to serve their needs.

Develop and implement a strategy to ensure that the organization and client’s needs are met through talent management, employee engagement, total rewards, learning and development, and career and performance management

POSITION OVERVIEW

The Account Manager, Portfolio Management contributes in updating the Bank’s loan portfolio by processing service requests and annual reviews required during the life cycle of a loan. Actively address situations where potential risks are identified, such as arrears, risk deterioration and declining financial results

Evaluate high and advanced risk requests and offer proactive solutions to restructure agreements, minimize risk and mitigate losses in collaboration with Credit, the business centers and other internal colleagues

Authorize, decline or recommend services, within the established parameters, and ensure a high caliber of analysis and due diligence for all loan amendments and service requests

Maintain relationships with business center personnel and colleagues to foster good teamwork and the pursuit of common objectives and the consistent application of policies

Coach, mentor and share experience with team members

Keep abreast of economic trends in his or her region and any potential impact for the Bank’s clients

Keep abreast of changes made to policies, procedures and detailed processes and of trends in credit and regulatory issues that may have an impact on BDC

WHAT WE ARE LOOKING FOR

Bachelor’s degree in Commerce or Business Administration

Three to five years’ experience as an account manager in the banking sector

In-depth knowledge of banking products, services, policies, processes and procedures

Ability to analyze complex financial structures

Ability to identify and solve problems pro-actively with good judgment

Demonstrated flexibility to adapt to a changing environment

Good time management skills and ability to set priorities to meet tight deadlines
