Job Duties

  • Performs, develops and interprets routine and/or specialized laboratory testing independently without error, within established time frames.
  • Performs, analyzes and records quality control and instrument maintenance/troubleshooting making judgments concerning results, and takes appropriate action necessary to maintain accuracy and precision. Escalates appropriately when results are out of control and ensures adequate documentation of corrective action.
  • Adheres to regulatory compliance guidelines and maintains an awareness of accreditation standards for laboratories by reviewing the most current checklist/standards. Follows safety guidelines, OIG compliance requirements and follows protocol for proficiency testing of samples.
  • Participates in performance improvement opportunities by assisting in the collection and analysis of data and supports business development and cost reduction initiatives by managing supply and reagent inventory.
  • Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

  • The successful candidate will support operational delivery by demonstrating and understanding key performance indicators and business pressures.
  • The successful candidate will need to engage in annual GMC appraisal and re-validation and hold a licence to practice.

Summary OF JOB RESPONSIBILITIES:

The Medical Social Worker prepares social histories, provides social casework to patients and families having difficulty in social functioning primarily as it affects their health, refers and aids in the referral process to social agencies, and generally serves as a social worker in the Agency’s total program. The Medical Social Worker implements standards of care for medical social work services.

The Medical Social Worker will assess and evaluate all Agency referrals directed to him/her and will devise a plan of care in accordance with Agency policies and priorities.

GENERAL:

The social casework service is to augment the other services being provided by the Agency. Primarily it is intended to help the nurses provide more comprehensive public health nursing service and to provide direct casework service to the patient or family until the problem can be resolved or a referral made to an appropriate agency.

DUTIES AND RESPONSIBILITIES: * Provides social casework as an entity where appropriate to individuals and families receiving nursing or other service from the Agency.

  • Performs complete psychosocial, emotional, safety, spiritual, terminal care assessment.
  • Prepares social histories taking into consideration:
  • Family relationships
  • The meaning of illness to patient and his/her family
  • The cultural attitudes in relationship to illness
  • The ability of the patient and his/her family to adjust to the patient at home
  • The financial impact of illness upon the family.
  • Develops a problem list, plan of care, establishes and revises realistic goals, performs discharge planning, reviews plan of care prior to care and performs services per plan of care. Evaluates patient’s response to care and progress and revises realistic goals and plan of care as needed.
  • Assess coping skills of patient’s family and assist in building increased coping skills as needed.
  • Helps the patient and his/her family to understand, accept and follow medical recommendations that provide services planned to restore the patient to optimum social and health adjustment within his/her capacity. Monitor social and emotional status and provide supportive counseling.
  • Responds to referrals for casework by agency staff or by professionals from outside the agency, providing service as appropriate.
  • Maintains up-to-date, concise, legible and timely documentation such as incident reporting, orders for services, summary reports and discharge paperwork for each patient receiving social work services. This includes biweekly conferences with other professionals providing care of that patient along with participation in case management conferences. All documentation will reflect the skilled nature of visits.
  • Assures accuracy in documentation of patient care services provided and is aware of and participates in taking steps to prevent fraud and abuse.
  • Assures that visits are scheduled and made which meet patient’s care needs and are authorized by the physician’s plan of care.
  • Obtains timely initial authorization and reauthorization as required from all managed care plans.
  • Participates in case conferences with those involved in patient’s care, functions as a team member by supporting agency staff members.
  • Follows all infection control practices in providing care. Reports and documents key information to PCN, physician, discharge planner, case manager, supervisor, etc.
  • Provides information and/or refers patients and families to community agencies and serves as a liaison between each.
  • Identifies patient and family needs for other home health services and refers to same when appropriate
  • Provides assessment, counseling and referral for patients with suicidal risk and mental/physical abuse and neglect.
  • Assesses, when appropriate, a family’s financial situation, taking into consideration patient prognosis and medical needs, referring to an Agency for financial assistance when indicated.
  • Provides counseling for long-range planning and decision making. Arranges for long-term care.
  • Participates in alternative and appropriate discharge planning.
  • Interprets social resources to staff and health services to social agencies.
  • Participates in in-service education programs within or outside of agency.
  • Respects all patients’ individual rights regarding HIPAA.
  • Personally implements, promotes and enforces the Stratford VNA Corporate Values System.
  • Performs other duties as assigned by the Supervisor of Clinical Services.

Qualifications AND EXPERIENCE:

The Medical Social Worker shall be a graduate of a Master’s degree program in social work accredited by the Council on Social Work Education who has had at least three years of clinical social work experience in a health care setting, such as a hospital, outpatient clinic, medical rehabilitation facility, or family service agency licensed in the State of Connecticut as a Medical Social Worker (LMSW). Clinical Licensed Social Worker (CLSW) preferred.

KNOWLEDGE AND ABILITIES: * Knowledge of all social work methods with particular skill in casework.

  • Ability to adapt this knowledge and skill to a public health setting.
  • Some knowledge and understanding of the principles of public health administration and practice.
  • Ability to collaborate effectively with community agencies.
  • Knowledge of Medical/State and home health guidelines.

The Medical Science Liaison (MSL) will represent Sun Pharmaceutical (SUN) across the United States. The MSL will primarily identify and engage with a targeted group of national and regional thought leaders/healthcare professionals (HCPs) as well as payers and managed care accounts, providing the consistent delivery of educational and compliant scientific information in support of Oncology and Specialty products. The MSLs will engage with HCPs, NPs, & PAs who are in clinical practice caring for their patients. They will be trained to respond to complex inquiries in a scientific, fair-balanced, compliant manner serving as a critical field medical resource to these HCPs and also, internal SUN sales and managed markets constituents.

Duties and Responsibilities

  • Interact with healthcare providers and thought leaders to communicate and advance the scientific platform as aligned with SUN’s corporate goals and objectives.
  • Communicate complex scientific information and research concepts to HCPs
  • Provide clinical and economic value information to payers and formulary decision makers
  • Identify and train members of SUN’s speakers’ bureau and ensure they are updated on new data
  • Respond to requests for investigator sponsored research (ISS) proposals and transit them into Sun and present the research to Sun for review. They will become the point of contact with the HCPs if ISS research is accepted and funded by Sun
  • Support SUN sponsored research
  • Provide feedback based on field interactions with healthcare providers and Thought Leaders to SUN stakeholders
  • Respond to scientific inquires consistent with the MSL compliance standards,
  • Foster scientific relationships between SUN and Fellows in training
  • Lead and/or participate on committees or project teams that support MSL strategies and tactics as delegated by MSL leadership
  • Seek out opportunities to support the MSL value proposition, achieve results and set an example for others to follow by consistently demonstrating SUN values and leadership attributes
  • Must have the ability to gain customer feedback, uncover business opportunities for SUN and pair customer’s unmet needs with available internal resources.

Qualifications

  • Doctorate / Terminal degree required (eg, PharmD, MD, DO, DNP or PhD)

  • Thorough grasp of the pharmaceutical industry, FDA requirements and PhRMA code
  • Strong interpersonal, organizational, team, written and verbal communication skills

The presently-anticipated base compensation pay range for this position is $130,000 to $159,000. Actual base compensation may vary based on a number of factors, including but not limited to geographical location and experience. In addition, this position is part of the Annual Performance Bonus Plan. Employees are eligible to participate in Company employee benefit programs which include medical, dental and vision coverage; life insurance; disability insurance; 401(k) savings plan; flexible spending accounts; and the employee assistance program. The Cardiff Eye Unit based at University Hospital of Wales provides a comprehensive ophthalmic service for Cardiff and Vale and surrounding areas. The department also contracts with purchasing authorities in most other parts of South Wales.

Cardiff and Vale University Health Board is recognised as a teaching centre of excellence in a wide range of clinical services.

You will be able to take advantage of the excellent Consultant contract for Wales. The basic salary range for this post is £91,722 per annum to £119,079 per annum plus a further £26,672 per annum available in Commitment Awards. Consultants in Wales paid on one of the first three points of the substantive Consultant pay scale also receive a Wales recruitment incentive payment.

Applicants must ensure they provide names and contact details of referees covering at least the last 3 years of their employment/training history in full. In addition, applicants who are currently or have most recently been employed as a substantive Consultant or as a Locum Consultant in post for more than 12 months must also provide the name of their Medical Director as an additional referee.

Main duties of the job

The appointee will join an established team of 17 Consultant Surgeons providing a comprehensive and high quality patient care service across all ophthalmology practice.

Cardiff and Vale UHB is committed to expanding the Consultant cover in the Ophthalmology service in the near future. There are excellent training and career development opportunities in the department.

Although this post is full-time, consideration will be given to applicants who require flexible working arrangements.

Applicants should either be on the Specialist Register or be a Specialist Registrar within six months of their expected date of receipt of a CCT / CESR(CP) at the time of interview.

Academic interests will be encouraged research and teaching in collaboration with Cardiff University’s School of Medicine will be expected.
