About Role :

• The position is linked to the delivery of statutory services: care and support needs assessment, carer’s assessment for support of individuals / residents (service users and carers) in the community, hospital, and residential settings within the legal framework of the Care Act 2014. The post holder will support the most vulnerable residents that require support from ASC to ensure eligible needs are met and they are safe and protected from abuse and neglect and will be required to facilitate hospital discharge and implementation of new operating model that brings discharge functions into the community SW teams.

The incumbent assists Veterans and family members with applications for long term care insurance coverage and serves as the point of contact that facilitates complex discharge plan coordination. The DPC social worker coordinator also collaborates with acute care and outpatient care DPC social workers on specific cases to offer guidance surrounding long term care standards of care specific to dementia care, behavioral disturbances, use of restraints, and conservatorship or legal matters. The incumbent serves as a consultant for un-insured and under-insured Veterans throughout the health care system. Services are at the advanced practice level and include high quality case management, coordination, and linking VA and community providers to achieve successful placement outcomes.

The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program’s budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program’s resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. STUDENT SUPPORT LEADER

Maternity Cover from February 2024

We are seeking to appoint an enthusiastic, highly motivated and inspirational Student Support Leader to join a dedicated pastoral team. This post is suitable for an experienced individual with a passion for promoting student success and well-being. This is a non-teaching (support staff) role and is to cover maternity leave, in the first instance.

The postholder will be responsible for the pastoral well-being of the students within the designated year group(s), undertaking all roles of behaviour, safeguarding, attendance, well-being, rewards, and pupil premium for this group of students. Excellent communication skills are an integral part of this role.

The Community Drug and Alcohol Team in Swansea is looking for an enthusiastic Consultant Psychiatrist to join our service, develop close links with Tier 2 agencies and provide expertise to the substance use liaison service in the general hospital.

Accepting this role will see you joining a very experienced team of nurses and medical staff to ensure that individuals experience a seamless and co-ordinated approach to their care. We welcome experienced applicants in addictions/substance use and final year CCT trainees within 6 months of completion.

Main duties of the job

• Provide clinical expertise and leadership to the substance use multidisciplinary team and the clinical management of patients with substance use problems.
• Provide supervision for junior doctors and trainees.
• Provide supervision for the Advanced Nurse Practitioner/Non Medical Prescriber.
• Provide cross cover duty for Consultant in Neath and Port Talbot for annual leave/study leave.
• Liaise effectively with members of the team, other services within the general hospital, general practitioners and other agencies.
• Contribute to the management of the service by attendance at appropriate committees and meetings, by involvement in audit and by taking on specific management roles which may, from time to time, be negotiated.
• Play an active role in the education and training of junior medical, nursing and other staff.
• Take part in the consultant on-call rota which is 1 in 20 across Swansea and Neath, Port Talbot.
• Undertake responsibilities under the mental Health Act as a s.12 approved doctor and as an Approved Clinician. Undertake responsibilities under the Mental Health (Wales) Measure.
• Support the Liaison Nurse based in Morriston District General Hospital

• Identify and recommend opportunities and develop strategies to acquire internal and external clients through trade shows, community events, culture and social events and industry related meetings.
• Hands-on approach to manage all activities and oversees the execution of events

Qualifications:

• Minimum of 2- year of corporate, conference center or hotels sales and/or events experience. Strong marketing background preferred
• Ability to work a flexible schedule including occasional evenings as needed. This is an in-person position, not a hybrid or remote role. Time will be divided between the NJ and NY offices.
• Effective negotiating skills managing outside vendors’ relationships.
• Excellent oral and written communication skills.
• Excellent organizational skills, with the ability to work under pressure, prioritize, and multitask.
• Strong computer skills and knowledge of office technology / equipment. Knowledge of EMS is a plus
• Discreet, ethical and committed to maintaining a high degree of confidentiality.
• Punctual, dependable and dedicated to achieving excellence, down to the smallest of details.
• Experience in conducting post-implementation discussions with client management and other team members to determine satisfaction levels.

Education:

• Four- year college degree preferred, specifically related to guest, facilities or Hospitality Services.
