Job description: Luton Community Mental Health Team (North)

This is an established post into the Luton Community Mental Health Team Structure. The post will complement the current structure of consultant and trainee psychiatrists within the community mental health teams covering Bedfordshire and Luton.

Luton community mental health teams are undergoing a process of redesign in line with the national community mental health transformation programme in order to focus on providing prompt assessment and intervention at the point of need within a new primary care-linked assessment service.

The post holder will support the multidisciplinary/multiagency team in the management of patients with mental health needs not on CPA and those patients subject to CPA, as envisaged in relevant NICE Guidance (at present). The post-holder will work with an MDT/multiagency including pharmacy, psychology, social work, CPN colleagues, VCSE colleagues and trainees. Post holder will clinically lead the Team, overseeing the delivery of care under CPA and non-CPA, for a cohort of patients registered with certain primary care clinical networks.

Main duties of the job

The post-holder will: * Provide consultant leadership to the Luton Community Mental Health (CMHT) North team.

  • Provide comprehensive assessment, diagnosis and management of issues relating to mental and physical health for adults with acute and severe mental illness in the community (over 18 years) so that intensive and timely treatment can be delivered to prevent chronicity and dependency on psychiatric services.
  • Attend and be the clinical leader of weekly MDT meetings, supported by the team operational lead.
  • Provide clinical advice and guidance to primary care physicians to manage patients in the least restrictive environment
  • Undertake all work in the context of the recovery approach which is emphasised across the Trust, placing the service user at the centre of planning and decision making.
  • Fulfil Responsibilities as a Psychiatrist approved under Section 12 of the Mental Health Act 1983, where relevant.
  • The service has a KPI targets and a requirement to meet them. The post holder will work with the Team manager to ensure the targets are met.
  • Provide support and training to other health care professionals and support service users through the care pathway as required
  • Lead on the delivery of high quality care in all settings and be mindful of quality targets set by the Trust and those set nationally.
  • Develop the service with a view to achieving HTAS accreditation at the earliest opportunity.

Job description: Applications are invited for the post of Consultant Psychiatrist within the North West area of Cardiff.

The post holder will act as Responsible Clinician to clients cared for by the neighbourhood services who are in-patients or in the community under the Mental Health Act 1983 and in accordance with the Wales Mental Health Measure.

The appointee will manage both community and in-patients but they are supported by a Consultant colleague in the Crisis Service who manages the early part of acute admissions (up to 14 days). North-West (Gabalfa) CMHT covers a diverse part of Cardiff with some areas of deprivation and serves an adult population of approximately 95,000.

You will be able to take advantage of the excellent Consultant contract for Wales. The basic salary range for this post is £91,722 per annum to £119,079 per annum plus a further £26,672 per annum available in Commitment Awards. Consultants in Wales paid on one of the first three points of the substantive Consultant pay scale also receive a Wales recruitment incentive payment.

Applicants must ensure they provide names and contact details of referees covering at least the last 3 years of their employment/training history in full. In addition, applicants who are currently or have most recently been employed as a substantive Consultant or as a Locum Consultant in post for more than 12 months must also provide the name of their Medical Director as an additional referee.

Main duties of the job

The CMHT covers the neighbourhoods of Llanishen, Whitchurch, Llandaff North, Gabalfa, Lisvane, Heath and Cathays and is based from the Gabalfa Centre. A weekly multidisciplinary referral meeting is held where referrals are discussed and individual care pathways agreed. A weekly MDT is held where assessments are presented, complex cases discussed and in-patients reviewed. There are an average of 90 new referrals a month which are screened and allocated, with the majority being seen initially by non-medical members of the team or signposted to appropriate agencies. The number of in-patients vary and are currently accommodated in the new purpose-built Acute Adult Inpatient Unit at Hafan Y Coed, University Hospital Llandough and which also has a high care ward where patients may be accommodated if needed.

Cardiff and Vale University Health Board is recognised as a teaching centre of excellence in a wide range of clinical services.

Although this post is full-time, consideration will be given to applicants who require flexible working arrangements.

Applicants should either be on the Specialist Register or be a Specialist Registrar within six months of their expected date of receipt of a CCT / CESR(CP) at the time of interview.
