OVERVIEW

Applications are invited for a Registrar / LAS SpR / Speciality Doctor / StR4+ in Acute Medicine. The post will commence as soon as possible (ASAP). This post offers the opportunity to develop general registrar level skills and possibly more advanced skills for those interested and those doctors with the appropriate experience to do so.

GENERAL DUTIES (guideline only)

· Appropriate clinical responsibility for the care of patients delegated by the consultant to his/ her charge. Co responsibility for the proper functioning of the Acute Medicine department, including the supervision of more junior medical staff.

· Participation in the Registrar on call rota and to provide cover in exceptional circumstances for colleague’s absence.

· Attending clinics and assessing patients and formulating treatment decisions seeking advice from the consultant as appropriate.

· Attending sessions with the consultant and assisting or performing procedures as appropriate.

· Running of wards with consultant cover on site during the day, consultant off site after hours.

· Participation in clinical audit.

· Attendance at clinical meetings and case conferences as appropriate.

· Working within a multidisciplinary team.

· Teaching and training staff in other medical disciplines, nursing and support staff as appropriate.

· Communication with other team members as appropriate.

· Co-ordinating appropriate medical disciplines concerned with care for your speciality and communicating appropriately with general practitioners (GP’s) and community agencies.

· Assessment, care and treatment of patients presenting acutely to the speciality and referred to the speciality.

· Conducting daily ward rounds on inpatients under the consultant’s care, supervising the junior doctors whilst the patient is in their care.

· In conjunction with the admitting clinical teams and the wider health economy to develop and implement integrated Acute Medicine pathways throughout the hospital / trust.

· Participation in relevant audit for the department and quality assurance procedures.

· Abide by the clinical & Corporate Governance policies of the hospital/ trust.

· To make sure that the guidance from the GMC (General Medical Council) on Good Medical Practise is at the heart of your clinical practise.

· To keep up to date with Acute Medicine knowledge by Continuous Medical Education in the best interest of the patient and your personal development.

· Although a timetable will be provided, you will be expected to be flexible, in the interest of patient care and to meet the needs of the department as decided by the general manager and clinical lead for your department – Acute Medicine.

· To contribute to the wider corporate and organisational needs of the hospital / trust as appropriate.

In addition doctors will be required to:

· Adhere to hospital / trust procedures e.g. Health and Safety at Work, Equal Opportunities policies etc.

· Work to the hospital/ trusts values- Put Patients First, Take Ownership, Respect Others, Be Positive.

· Maintain personal and professional development to meet the changing needs of the job, to seek medical education as appropriate and encourage and support staff development and training as appropriate with your grade.

· Attend Statutory, essential and mandatory training.

· Respect the confidentiality of all matters relating to their employment and other members of staff. All Members of staff are required to comply with the requirements of the data protection act 1998.

JOB PLAN

To be discussed with the Clinical Lead of Acute Medicine and departmental service manager – all job plans are made with the agreement of the doctor and needs of the Acute Medicine department.

SALARY

The salary is based on the national salary scale as outlined in the Medical and Dental Terms and Conditions – £45,124 to £77,519 per annum not inclusive of extra hours, these are paid in addition to basic salary.

PERSONAL & PROFESSIONAL DEVELOPMENT

The post-holder will be required to keep himself / herself up-to-date with their relevant area of practise and be able to demonstrate this to the satisfaction of the hospital / trust / GMC.

The trust supports the requirements for continuing professional development as set down by the Royal College’s and is committed to providing time and financial support for these activities, this is also the same for GMC Revalidation.

STUDY LEAVE

Professional or study leave is granted for purposes approved by the trust and includes time off for study, research, teaching, examining, taking examinations, visiting clinics external (if required) to the hospital / trust and attending professional conferences. Study leave allowance is a maximum of 10 days per annum.

  • Demonstrated knowledge of the assessment and management of co-morbid medical disorders (ie. diabetes, metabolic monitoring, etc.)
  • Demonstrated knowledge of Mental Status Examinations (MSE)
  • Demonstrated knowledge of individual counseling and group skills (ie. DBT, MI, IPT, CBT, Behavioral Activation)
  • Knowledge of risk assessment and management, including knowledge of trauma informed care and alternatives to seclusion and restraint, and associated risks

OVERVIEW

The hospital offers an excellent starting salary (dependent on experience) with pay enhancements for overtime, shift and out of hours work that you do, you average working week will be 39 hours per week plus overtime opportunities as and when required.

As a SHO/ Senior House Officer you will work closely with other junior doctors and a team of registrar doctors and Consultant within your department. You must maintain liaison with colleagues who are on duty immediately before and after you to ensure adequate communication and continuity of patient care. You will instruct nursing staff in the care of patients, also the professional technical staff in the investigations as required- those necessary for patient care and take patient care that other departments are fully acquainted with all necessary information.

  • You will refer to the Consultant(s) when problems arise which are beyond your authority and/or experience, and will oversee and advise the House Officers.
  • Applying specialist knowledge and skills across a range of impairments, procedures and practices gained through practical experience, underpinned by theoretical knowledge.

  • Providing diagnosis and treatment for all categories of patient in the department.
  • To actively participate in staff meetings, forums and development programmes in order to support development of service and maintenance of high quality care.

Austin Market Float Pool (AMFP) employees are required to float to all St. David’s HealthCare System Facilities. AMFP nurses will be scheduled in units in which they have competencies and can be placed in a unit below their competency level with a modified assignment.

  • You will perform and document patient assessments at the time of the admission
  • You will create a plan of care, including discharge plans, revise the plans based on the patient’s response to treatment, and regularly evaluate plans for effectiveness
  • You will document the outcomes of nursing interventions and patient responses
  • You will provide educational materials to patients and families on care and treatments
  • You will administer medications and complete treatments according to hospital policy
  • You will perform venipuncture, administer and maintain IV fluids and IV medications
  • You will review all new physician orders, ensure that orders are carried out and documented correctly, and verify that other departments are notified of orders and services
  • You will notify the Charge Nurse and/or physician of changes in patient status
  • You will provide accurate and relevant patient information reports to the on-coming shift to ensure continuity of care

Market Float Pool RNs are scheduled in accordance with the Staffing and Scheduling- Nursing, RN Floating – Healthy Workplace Environment (HWE), and Attendance policies. Requirements of RN unit-based, facility-based, or system-based pools are based on which pool they are in.

Hours:

Full Time: Minimum of 36 hours per week. Two weekends (or four-weekend shifts) per 4-week scheduling period is required. The weekend is defined as Saturday and Sunday.

Holiday Requirements:

One Major holiday for the first half of the year (January-June) and one Major holiday for the second half of the year (July-December). One Minor holiday for the first half of the year (January-June) and one Minor holiday for the second half of the year (July-December).

What qualifications will you need:

  • two years of nursing experience in two areas of hospital nursing patient care.
  • Associates Degree in Nursing (ADN) from an accredited Nursing program
  • Current State of Texas Registered Nurse license or Compact Registered Nursing License. If Compact License Texas license must be obtained within 90 days of hire
  • Basic Life Support (BLS) required as per St. David’s Healthcare policy
  • Advanced Cardiovascular Life Support (ACLS) certification within six months
  • National Institutes of Health Stroke Scale (NIHSS) certification required per St. David’s Healthcare Policy – within 60 days of hire
